Summary


This is a full-time non-exempt position that is responsible for providing behavioral health case management services to adult and/or child-adolescent clients and their families. Primary work hours will be Monday thru Friday, 8:30 am to 4:30 pm with a 1/2 hour lunch (37.5 hrs/week) unless otherwise agreed upon. This position is also responsible for the provision of crisis intervention services 302, and 201 hospitalization coordination and various other services as part of the Base Service Unit Team. Completion of all necessary paperwork per agency, county and state regulations is required.

Adheres to WCSI policies, BH Department procedures, and County, State, and Federal regulations governing Administrative Case management as well as Targeted Case Management.  Attends specially designed training programs provided through WCSI, county, state, and federal agencies.




Essential Duties and Responsibilities


Include the following, (other duties may be assigned as deemed necessary by management):

Case Management and Service Coordination:
Responsible for assessing the behavioral health and psycho-social needs of behavioral health clients. Referral, service linkage, community support networking and development and case monitoring are essential job duties. Documentation of all service management contacts are maintained in client charts. Liaison responsibilities include working in conjunction with various treatment programs; i.e. partial, outpatient, residential, etc.  Coordination and liaison responsibilities with other human services providers. Participation and attendance at interdisciplinary team meetings, utilization review, and treatment plan staffing. Completes all service activity documentation accurately and promptly. Responsible for achieving assigned services rendered rate.

Crisis Intervention/Hospitalization Coordination:
Works as a Base Service Unit Team Member. Provides crisis services including telephone, walk-in and mobile crisis services on an as-needed, assigned basis. Coordination of 302 and 201 hospitalization includes completion of all required legal documents and emergency phone coordination with county, local community hospitals, emergency medical services and police.

Related Service Management Activities:
Services are provided within the community, residential programs, and at the site.  Complete client intake and referrals as required by the Behavioral Health Department.   This position will also be trained in the duties of other Administrative Case Management, Liaison, and Targeted Case Management positions, with the ability to provide coverage, as assigned.

Obtain necessary training in accordance with assigned staff development requirements. Able to be flexible, adjust work hours based upon program and consumer needs as assigned by immediate supervisor. Display a professional standard of effective teamwork in the provision of Administrative Management Services. Perform related duties as assigned.



Supervisor Responsibility


This position has no supervisory responsibilities. The AM Case Manager reports directly to the TCM/COC/AM Supervisor and may take direction from the Behavioral Health Program Manager, Behavioral Health Program Director, and Chief Executive Officer.

Education and/or Experience:

Bachelor's degree in Social Service related field and previous case management experience preferred.

Language Skills:

Strong Interpersonal skills, effective oral and written communication skills.

Certificates, Licenses, Registrations:
Valid PA Driver’s License, Proof of Valid Auto Insurance, Act 34 (Criminal Record), Act 33 (Child Abuse) and FBI Clearance. There will be monthly screenings of PA Medicheck List, LEIE and SAM.

 

Physical Demands:
While performing the duties of this job, the employee is regularly required to operate a motor vehicle, sit, stand, walk, bend and lift objects of up to 10 pounds. The employee regularly uses a computer, phone, fax, copier and other essential office equipment to carry out daily work assignments.

Substantial problem solving in response to urgent/emergency situations, which may require immediate action.  Ability to apply principles of leadership and management in daily practice.


Work Environment:
While performing the duties of this job, AM Case Management is provided as needed in the place where the client resides or needs the service. Case Management is a community based service and offers service in the client's home, community center, and place of worship or treatment center. Case managers will provide linkage, monitoring, access to services; provide assessment and service planning, problem resolution and informal support network building for clients on their caseload in a client driven fashion where and when it is needed. The services of this job are community based. While performing the duties of this job, the employee is exposed to a combination of office and outside/travel conditions and regularly exposed to moderate levels of noise.  Exposure to outside weather conditions will exist.
