WCSI Position Title: Targeted Case Manager-COC

This is a full-time non-exempt position primarily responsible for the continuity of care for clients during extended time off of traditional TCM's.   Primary work hours will vary, but occur between Monday thru Friday, 8:30AM to 4:30PM with a ½ hour unpaid lunch period. Time will be flexible to meet client needs. Occasional weekend work may occur for special events. The Continuity of Care (COC) TCM will manage the cases of a peer who will be out of the office due to FMLA or other extended time off. The COC TCM responsibilities are consistent with traditional TCM: the assessment of client functioning and the development, preparation and implementation of a comprehensive Targeted Case Management service plan, however, the COC TCM is considered a floater and goes to the team where the caseload must be covered. The COC TCM will not be assigned a permanent caseload.  The COC TCM will provide necessary services, enabling clients to be served in a consistent and recovery oriented manner. Hours are full time, yet flexible, and travel to the assigned sites will be paid based on the Greensburg office being the base of operations. The COC TCM is responsible for monitoring all services received by TCM clients, resource development, service linkage, crisis intervention services and client advocacy. Works as an interdisciplinary team member. Work assignments will be discussed as part of regular supervision and assigned with the best interest of the clients and the department in mind. Goals and objectives will be developed and priority assignments identified. Maintains all TCM records and other relevant paperwork accurately and promptly.

Essential Duties and Responsibilities

Include the following, (other duties may be assigned as deemed necessary by management):

Compliance: Adheres to WCSI, state, county, and federal regulations governing Targeted Case Management service provisions and Department of Transportation regulations. Attends specially designed training programs provided through WCSI, state, county, and federal agencies.

Case Management Activities: Assess client need utilizing the Environmental Matrix and Strength Based Assessment (SBA), client and/or collateral interviews, and reviews of clinical and social information. Needs assessment will consider current and historical information, with emphasis on precipitating factors to deteriorated functioning.

Develops, prepares and implements a comprehensive Targeted Case Management Service Plan which includes short and long term goals/outcomes based on client, provider and TCM team input. Goals and outcomes are strength oriented and clearly operationalized to facilitate assessment of progress. Outcomes will be directed towards reduced hospitalization, independent living, improved vocational/educational status and social supports. Conducts regular, documented re-evaluations of client progress. Ensures face-to-face contact to the client based on program requirements, working on a variable schedule as assigned by supervisor.

Monitoring and Coordination Activities: Monitors all services received client by reviewing cases; meeting with family, significant others, relevant providers, advocates and attorneys; and attending TCM team meetings. Participates in interdisciplinary and psychiatric treatment team meetings as required. Ensures clients? needs are met through utilization of natural supports and other social services. Investigates new resources and communicates with respective providers on the client's behalf. Maintains current directory of community resources. Assists client with identification, linkage, access and coordination of these resources.

Advocacy Activities: Advocates for the client to ensure responsiveness from natural, community, generic and specialized services. Advocacy includes providing information, removing barriers, and creating options and resolving problems.

Case Documentation: Maintains Targeted Case Management records, charts contacts with clients and collateral contacts. Updates forms as needed. Reviews charts for compliance with regulations. Prepares for annual record audits by the county and state. Efficiently completes paperwork and other program related material. Consistently maintain service provisions in accordance with the department standard, as established by WCSI.

Professional/Technical Responsibilities: Extensive knowledge of theories, principles, and techniques related to service provided. Ability to establish/maintain effective working relationship with clients, associates and agency personnel. Knowledge of community resources and ability to facilitate coordination activities. Ability to prepare clear, concise reports and to communicate effectively, both orally and in writing.

Supervisor Responsibility

This position has no supervisory responsibility. The Caseworker 1 (TCM-COC) reports directly to a Casework Supervisor and may also take direction from the BH Program Manager, BH Director and/or the Chief Executive Officer.

Education and/or Experience: Associates Degree Human Service/Social Science and two years experience in direct contact with Behavioral Health clients. Previous Case management experience preferred.

Language Skills: Strong Interpersonal skills, effective oral and written communication skills.

Certificates, Registrations, Licenses: Valid PA Driver?s License, Proof of Valid Auto Insurance, Act 34 (Criminal Record), and Act 33 (Child Abuse) and FBI Clearance. There will be monthly screenings of PA Medicheck List, LEIE and SAM.

Physical/Mental Demands: While performing the duties of this job, the employee is regularly required to operate a motor vehicle, sit, stand, walk, bend and lift objects of up to 10 pounds. The employee regularly is driving a vehicle, utilizing a cell phone, GPS, computer, fax, copier and other essential office equipment to carry out daily work. Must be able to perform CPR/First Aid if needed. Substantial problem solving is needed to respond to urgent or emergency situations.

Work Environment: While performing the duties of this job, BH Case management is provided as needed in the place where the client resides or needs the service. Case management is a community based service and offers service in the client's home, community center, and place of worship or treatment center. Case managers will provide linkage, monitoring, access to services; provide assessment and service planning, problem resolution and informal support network building for clients on their caseload in a client driven fashion where and when it is needed. Exposure to outside weather condition will exist. The noise level in the work environment is usually moderate.
