Summary


This is a full-time non-exempt position where the essential job duties and responsibilities are to ensure continuity of services for all individuals receiving case management services at WCSI through the Intellectual Disabilities Program.  This will be done by reviewing all assigned Individual Support Plans and demographics in HCSIS along with all supplemental documentation that have been submitted by Supports Coordinators to determine if all criteria have been met for appropriate and timely submission to county office for approval and authorization.  This position will be responsible for completion of service notes in HCSIS that provide an ongoing record of activities on behalf of an individual related to the implementation of the Individual Support Plan and also provide beneficial background information for Individual Support Plan comments.  Primary work hours will be Monday through Friday, 8:30 am to 4:30 pm with a 1/2 hour lunch (37.5 hrs/week) unless otherwise agreed upon. The tasks listed above will be carried out within the principles and philosophy of Self-Determination, Positive Approaches, and Everyday Lives. This position will complete and maintain all records and other relevant paperwork accurately and promptly. This position will adhere to the philosophies of the agency as it applies to Quality Assurance.

The unique characteristic of this position is that it will act as lead on all issues relating to ISP approval and authorization.  This will include being liaison with county program specialists to resolve pending revision issues which will include on-going communication to management about reviews and additions to program protocols. 


Essential Duties and Responsibilities


Include the following, (other duties may be assigned as deemed necessary by management):

Compliance:
Adheres to WCSI, county, state, and federal regulations governing Supports Coordination service provisions which include Targeted Service Management and Waiver. Attends specially designed training programs provided though WCSI, county, state, and federal agencies.

Assessment:
Maintain knowledge and ongoing protocols concerning all assessment tools such as Prioritization of Urgency of Needs for Services, Personal Plan Information, Supports Intensity Scale (SIS), and Adaptive Behavior Assessment Scales, consumer and/or collateral interview, and review of educational, clinical, and social information.

Individual Support Plan (ISP):
This position will review all ISPs and demographics in HCSIS along with all supplemental documentation that have been submitted by SCs to determine if all criteria have been met for appropriate and timely submission to the county office for approval and authorization.  The ODP issued ISP Manual and “ISP Annual Review Criteria for Submission” form will be used for all plans and when necessary the “SC Supervisor Checklist for AE Review and Approval of the ISP”. All ISPs and attachments must meet the appropriate timelines for submission.

 


Home and Community Service Information System (HCSIS):
Possess the necessary computer skills in order to successfully navigate the web-based system. Document in HCSIS case note format all case activity for a particular individual.


Advocacy:
Advocates for the consumer to ensure responsiveness from natural, community, generic, and specialized services. Advocacy includes providing information, removing barriers, and creating options for resolving problems.

Documentation:
Service note documentation in HCSIS will be required to provide an ongoing record of activities on behalf of an individual with relation to the Individual Support Plan review and submission.  Service note documentation will provide information relating to Individual Support Plan comments or concernsMaintain records and document contacts concerning consumers. Maintains documentation for compliance and regulations. Completes paperwork and other program related material in a timely and efficient manner meeting all program standards. Consistently maintain service provisions in accordance with the department standard as established by WCSI.

Professional/Technical Responsibilities:
Extensive knowledge of theories, principles, and techniques related to service obtained by attending necessary training and accessing current information in the field. Possess the ability to establish and maintain effective working relationships with consumers, associates, and agency personnel. Possess knowledge of community resources and have an ability to facilitate meetings and professionally represent the agency. Act as back-up for all Complex Technical Assistance responsibilities and activities. Also act as liaison with county program specialist to resolve pending revision issues which will include on-going communication to management about reviews and additions to program protocols.

Coverage/Primary Back-up Duties:

When determined to be necessary by the IDD Program Manager or IDD Program Director this position will provide coverage or act as primary back-up for all job duties normally assigned to Continuity of Services Supports Coordinators when necessary. 

 

Other Duties

This position may be required to perform other duties, as may be assigned by a Program Supervisor or management staff.



Supervisor Responsibility


This position has no supervisory responsibilities. The COS will serve as a direct resource for support coordinators and their supervisors. This position will provide assistance to Support Coordinators and Supervisors as regulated by the Office of Developmental Programs on Individual Support Plan creation and submission.  The COS reports directly to the ID Program Supervisor and may take direction from the IDD Manager/Director and/or the Executive Director.

Due to the nature of this position, the incumbent must have strong time management skills, computer skills, and organizational skills.  The incumbent must be highly flexible to meet the needs of the department and the consumers.  The incumbent must present the ability to work independently, with minimal supervision. 


Education and/or Experience:
Bachelor's degree in Human Services or other related field, with minimum three (3) years of case management experience which includes one(1) year as Supports Coordinator with Westmoreland Casemanagement and Supports, Inc. (WCSI) preferred.

Language Skills:
Strong Interpersonal skills, effective oral and written communications skills. Ability to prepare clear and concise reports and documentation.

Certificates, Licenses, Registrations:

Valid PA Driver’s License, Proof of Valid Auto Insurance, Act 34 (Criminal Record), and Act 33 (Child Abuse). There will be monthly screenings of PA Medicheck List, LEIE and SAM.

Physical /Mental Demands:
While performing the duties of this job, the employee is regularly required to operate a motor vehicle, sit, stand, walk, bend and lift objects of up to 10 pounds. The employee regularly uses a computer, phone, fax, copier and other essential office equipment to carry out daily work assignments.

Ability to apply principles of leadership in daily practice and ability to problem solve in response to urgent/emergency situations, which may require immediate action. 

Work Environment:
The services of this job are community based. While performing the duties of this job, the employee is exposed to a combination of office and outside/travel conditions and regularly exposed to moderate levels of noise. Exposure to outside weather conditions will exist.
