Summary


This is a full-time non-exempt trainee/entry level position as a Supports Coordinator. This position is the first series of positions that encompass the one (1) year introductory period for the Supports Coordinator series. 

 During the initial training period of three (3) months, required for the Trainee position, the employee will learn to perform ongoing assessment of consumer needs and the development, preparation, and implementation of a comprehensive Individual Support Plan through the initial training period.   This position also learns the necessary skills to locate, coordinate, and monitor services that include incident management, service linkage, crisis intervention, and consumer advocacy incorporating the principles and philosophy of Self-Determination, Positive Approaches, Everyday Lives and the agency’s quality assurance principles.  

 A Trainee position does not complete independent functions that would produce billable service provisions.

After successful completion of a mandatory three (3) month training period, the employee will be transitioned from the Trainee position to a Supports Coordinator-Intro/Caseworker 1 position, continuing an introductory period for the next nine (9) months.  After successful completion of the one (1) year intro period, position will be classified as a Supports Coordinator/Caseworker 1.  Consideration will be given at the completion of 2 years of continued successful employment as a Caseworker 1 to be reclassified to a Supports Coordinator/Caseworker 2. 

 Responsibilities for these positions are outlined in the appropriate position descriptions, as attached.

 Inadequate or marginal performance during the initial three month training period will result in automatic termination. 

 Primary work hours will be Monday through Friday, 8:30 am to 4:30 pm with a 1/2 hour lunch (37.5 hrs/week).

 As with all WCSI positions, it is the expectation that employees adhere to WCSI, County, State and Federal guidelines, regulation, policies and laws which govern the business and service of WCSI.


Essential Duties and Responsibilities


Include the following, (other duties may be assigned as deemed necessary by management):

:

Training and Orientation:

Attends all required training and orientation as assigned by ODP, WCSI and ID Program. Employee will receive weekly supervision.

Assessment:
Learns to assess consumer needs, utilizing assessment tools such as Prioritization of Urgency of Needs for Services, Personal Plan Information, and Adaptive Behavior Assessment Scales, consumer and/or collateral interview, and review of educational, clinical, and social information. Needs assessment will consider current and historical information.
Individual Support Plan:
Learns to develops, prepares, and implements a comprehensive Individual Support Plan which includes outcomes based on consumer needs and desires, provider and team input. Outcomes are strength oriented and all services are linked to an outcome.

Regulations:
Learns to adhere to Targeted Service Management regulations including a current comprehensive plan must be completed and updated at least annually. Learns to adhere to Waiver Regulations that include the Individual Support Plan and Monitoring Requirements.
Locating, Coordinating, and Monitoring:
Learns responsibilities that include assessing needs, making necessary referrals for services and for community/generic resources, and monitoring the provision of services and the overall health, safety, and satisfaction of the individuals whom they serve, including problem solving techniques as well. Learns to complete service preference when applicable, make referrals for Behavioral Health Services, and update the POMS Consumer Registry and Quarterly Update Status Forms. Learns to complete necessary authorizations when appropriate.
Home and Community Service Information System (HCSIS):
Learns the necessary computer skills in order to successfully navigate the web-based system. Learn to develop the Individual Support Plan including outcome development. Learn to complete critical revisions and budget information. Learn to complete necessary monitoring requirements per regulations and enter the monitoring information into HCSIS. Learn to document in HCSIS case note format all case activity for a particular individual. Learn to complete and update demographic information for each individual. Learns Incident Management which includes recognizing an incident, ensuring health and safety of the individual, completing necessary reports in HCSIS, and monitoring the follow through surrounding a particular incident.
Advocacy:
Learns to advocate for the consumer to ensure responsiveness from natural, community, generic, and specialized services. Advocacy includes providing information, removing barriers, and creating options for resolving problems.
Documentation:
Learns to maintain records and document contacts with consumers.  Learns to maintain documentation for compliance and regulations. Learns to complete Waiver Packets when funding is identified and also maintaining re-certification documentation annually. Learns to complete paperwork and other program related material in an efficient manner.  Learns to consistently maintain service provisions in accordance with the department standard, as established by WCSI.



Supervisor Responsibility


This position has no supervisory responsibilities. The Supports Coordinator reports directly to the IDD Supervisor and may take direction from the IDD Director and/or the Executive Director.



Qualifications


:

Education and/or Experience:

Associates Degree in Human Services or other related field which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences with a minimum one (1) year work experience in BH/IDD.

 Language Skills:
Strong Interpersonal skills, effective oral and written communications skills. Ability to prepare clear and concise reports and documentation.

Certificates, Licenses, Registrations:

Valid PA driver's license, Proof of valid Auto Insurance, ACT 33 (Child Abuse), ACT 34 (Criminal Record), FBI Clearance. There will be monthly screenings of PA Medicheck List, LEIE and EPLS.

Physical/Mental Demands:
While performing the duties of this job, the employee is regularly required to operate a motor vehicle, sit, stand, walk, bend and lift objects of up to 10 pounds. The employee regularly uses a computer, phone, fax, copier and other essential office equipment to carry out daily work assignments.

Substantial problem solving in response to urgent/emergency situations, which may require immediate action.  Ability to apply principles of leadership and management in daily practice.


Work Environment:
The services of this job are community based. While performing the duties of this job, the employee is exposed to a combination of office and outside/travel conditions and regularly exposed to moderate levels of noise.  Exposure to outside weather conditions will exist.

Other:

The incumbent selected for this position will be required to pass a pre-employment drug screen, as per WCSI’s Drug Awareness Policy.
